Introduction

Head and neck surgery and surgical oncology program aim to provide exposure to
the training fellows on the principal management of head and neck tumours, in
addition to the assessment and diagnosis of head and neck tumours. General
component of head and neck surgery and its related otorhinolaryngology diseases is
also a core of the programme.

Objectives of the program

The objectives are;

  • To expose the fellows to the most common head and neck surgery procedures
  • To expose the fellows to common head and neck pathology and its relation to general otorhinolaryngology disease
  • To expose to the fellows and provide experience in the management of head and neck tumors, especially the surgical procedures
  • To facilitate the fellows to be able to perform the clinical assessment, staging the tumors and perform most head and neck surgical oncology procedures
  • To expose the fellow to the multidisciplinary roles of plastic reconstructive, pathologist, radiologist, oncologist, and rehabilitation team in managing head and neck oncology patients.
  • To produce a highly competent head and neck surgeon practicing a sound surgical oncology practice

 

The institution USM provides training in the listed types of surgery below:

Core procedures

  • Excision biopsy of neck nodes and excision of neck masses
  • Incisional biopsy of tonsillar/tongue/buccal mass/nasal/nasopharynx
  • Direct Laryngoscopy, Bronchoscopy and Esophagoscopy & targeted biopsy
  • Tracheostomy under LA/GA
  • Submandibulectomy
  • Superficial parotidectomy
  • Partial glossectomy
  • Sistrunk’s operation
  • Resection of buccal/tongue/laryngeal leukoplakia
  • Stomal stenosis revision & stomaplasty
  • Excision of branchial cyst
  • Tracheoesophageal puncture & voice prosthesis insertion
  • Cricopharyngeal myotomy
  • Hemithyroidectomy & lobectomy

Specialized procedures

  • Total conservative parotidectomy with facial nerve preservation, radical parotidectomy and extended total parotidectomy
  • Total thyroidectomy & subtotal thyroidectomy
  • Total glossectomy & hemiglossectomy
  • Laryngectomy, hemilaryngectomy, supracricoid laryngectomy
  • Selective neck dissections (supraomohyoid, lateral, anterolateral and posterolateral type)
  • Modified Radical Neck dissection Type 1, II, III
  • Radical neck dissection
  • Mandibulotomy – Lateral & Median mandibolotomy
  • Mandibulectomy – marginal and segmental mandibulectomy
  • Lip split and lower cheek flap
  • Maxillectomy – Total and subtotal maxillectomy
  • Local flap – sternocleidomastoid flap, frontonasal flap
  • Repair of pharyngocutaneos fistula
  • Vagal schwannoma excision
  • Split skin graft and full thickness skin graft

  • Oncogenic viruses in head and neck oncology
  • Principle of laser surgery for head and neck pathology
  • Principle of imaging of choice in head and neck malignancy
  • Role of frozen section in head and neck malignancy

  • Metastatic carcinoma of unknown primary
  • Thyroid malignancy management
  • Salivary glands malignant tumour classification and principle of management
  • Principle of distant metastases of head and neck tumours
  • Classification of head and neck tumours and TNM staging classification
  • Principles of chemotherapy in head and neck malignancy
  • Principles of radiation in head and neck management
  • Principle of common types of flaps in head and neck
  • Salvage neck dissection
  • Adjuvant treatment for head and neck malignancy
  • Current advances in head and neck oncology
  • Management of recurrent head and neck tumours
  • Controversial and updates in head and neck surgical oncology

The programme duration: (2 weeks minimum, 6 months maximum)

Fellows will be supported to settle down into their new environment. A desk space
will be allocated. Full induction (safety, laboratory practices, personal welfare) will be
provided by the University’s designated staff and by myself. I will arrange an initial
meeting with the fellow to discuss the objectives of the training.

The fellow will be fully trained in all activities of the Department. They will be able to
engage with me (supervisor) and other members of the group in different surgical
procedures. They will also be engaged in clinical research and write a manuscript
during their stay depending on their interest.

The fellow will be given the opportunity to actively participate in group meetings and
other academic activities within the School and the University, including attending
workshops, conferences, etc. They will also be encouraged to present their work
orally at a group meeting at least once before they complete their visit.

Towards the end of the training, I will have a meeting with the fellow to discuss plans
to publish their clinical work (if enough data collected) or disseminate their findings at
a future conference.

Throughout the training, any particular challenges or specific requests of a fellow will
be fully considered to ensure a responsive, relevant and adequate training plan is
developed for them.

Language: English as main medium for teaching and surgery

This program allows Merit Applications. This program allows merit-based applications for virtual and onsite clinical and research programs. If you are successfully awarded under this category, Trialect or the host mentor will cover the tuition fee only. All applications will be evaluated based on merit. Due to the high level of competition, the chances of being selected under the merit category are quite limited.

Candidate with MBBS from a certified university Master of Otorhinolaryngology or Master of Plastic Surgery or Master of Oral Maxillofacial or equivalent degree. Experience as ENT surgeon for at least 6 -12 months
