Dr. Carlos Ferrando, MD, PhD, is a highly accomplished specialist in anesthesiology and critical care, currently serving as the Head of the Surgical Intensive Care Unit at the Hospital Clínic de Barcelona in Spain. He is also actively involved in research at the Centro de Investigación Biomédica en Red (CIBER) in Respiratory Diseases, which is part of Spain’s leading biomedical research initiatives. His expertise spans respiratory physiology, mechanical ventilation, and intensive care, particularly in managing critically ill surgical and respiratory patients.

This LIVE INTERACTIVE ONLINE traineeship will focus on several key areas that are essential for the understanding and management of respiratory support in critically ill patients:

  • Respiratory Physiology: Understanding gas exchange, the ventilation-perfusion ratio, alveolar ventilation, and dead space is critical for optimizing mechanical ventilation.
  • Mechanical Ventilation Modes: A detailed exploration of both invasive and non-invasive ventilation, including pressure-controlled, volume-controlled, and pressure support ventilation.
  • Lung Protective Ventilation: Emphasizing techniques that prevent ventilator-induced lung injury, such as low tidal volume strategies and optimal PEEP settings.
  • Advanced Non-invasive Respiratory Support: Focus on High Flow Oxygen Therapy (HFOT) and Non-Invasive Mechanical Ventilation (NIV) as alternatives to invasive mechanical ventilation.

Participants will acquire  skills in managing respiratory support across various clinical settings:

  • Interpretation of Ventilatory Mechanics: Analyze parameters such as peak pressure, plateau pressure, and driving pressure to optimize ventilation for different patient scenarios.
  • Implementation of Mechanical Ventilation Modes: Learn to set up and modify pressure-controlled and volume-controlled ventilation, as well as pressure support ventilation in both the operating room and ICU.
  • Application of Non-Invasive Techniques: Develop the ability to appropriately use HFOT and NIV to support patients with respiratory insufficiency, reducing the need for intubation.
  • Lung Protection Strategies: Master the use of PEEP and recruitment maneuvers in critically ill patients to prevent lung injury, while incorporating strategies like prone positioning and the use of neuromuscular blockers.

The traineeship consists of five comprehensive modules, each addressing different aspects of mechanical ventilation and respiratory support:

Module 1: Respiratory Physiology Applied to Mechanical Ventilation I

  • Ventilation-perfusion ratio and oxygenation
  • Alveolar ventilation and dead space

Module 2: Respiratory Physiology Applied to Mechanical Ventilation II

  • Basic ventilatory mechanics: Peak pressure, plateau pressure, driving pressure

Module 3: Mechanical Ventilation I

  • Overview of pressure-controlled and volume-controlled ventilation
  • Basics of pressure support ventilation (PSV)

Module 4: Mechanical Ventilation II

  • Non-invasive mechanical ventilation (NIV)
  • High Flow Oxygen Therapy (HFOT)

Module 5: Introduction to Lung Protective Ventilation

  • Lung-protective strategies: tidal volume, PEEP, and recruitment maneuvers
  • Adjuvant strategies: prone position, drugs (neuromuscular blockers, corticosteroids)

At the end of the traineeship, participants will be equipped to:

  1. Understand and Analyze: Analyze and interpret respiratory physiology and gas exchange during mechanical ventilation, including identifying abnormalities.
  2. Manage Respiratory Failure: Detect clinical signs of respiratory failure and select appropriate mechanical ventilation parameters for different clinical conditions.
  3. Optimize Ventilation Strategies: Apply lung-protective ventilation techniques, particularly in critically ill or surgical patients, to improve patient outcomes.
  4. Deploy Advanced Non-Invasive Techniques: Use and manage High Flow Oxygen Therapy and Non-Invasive Mechanical Ventilation for patients who require less invasive support.
  5. Apply ARDS Strategies: Develop effective strategies for the management of acute respiratory distress syndrome (ARDS), ensuring the use of evidence-based approaches to minimize injury and maximize recovery.
