The The First Orthopaedics Clinic at the Istituto Ortopedico Gaetano Pini routinely treats several patients for knee, hip, elbow, ankle and shoulder diseases at an high quality level with international recognition. This department is one of the most known in the country for Sports Traumatology. The department is under the direction of Professor Pietro S. Randelli. Staff members are almost 16, all experienced but the trainees will be in contact especially with Dr. R. Compagnoni (Knee-shoulder and trauma), Dr. C. Fossati (shoulder), Dr. P. Ferrua (Knee and Patellofemoral expert) and 6 residents plus several students.
A biomechanical lab managed by Prof. Pietro Randelli and Ph.D. Biology Dr. Alessandra Menon is in charge to conduct several clinical and research studies, around 6 per period. Research and clinical work are always ongoing.
Surgeries are performed 5 days a week, 8 am to 8 pm.
We perform Total Joints Arthroplasty (hip-knee-shoulder) and arthroscopy of all joints.
The outpatient clinic is open four days a week.

Arthroscopy of the knee (n=40);
Arthroscopy of the shoulder (n=20);
Arthroscopy of the elbow (n=5);
Adult joint reconstruction of the knee hip shoulder and elbow: (n=50);
Hand surgery: (n=20);
Orthopedic trauma: (n=60);
Foot and ankle surgery: (n=30);
The fellowship program, limited to 3 positions per period, lasts from 1 months to 1 year.

"Professor Pietro S. Randelli is a kind, generous human being and master in his field. I have never felt alone since I have been here in Milan; he is always there to help. He is interactive and always enthusiastic to share his knowledge. He is empathetic to his patients and fellows. He offered me cheap accommodation and provided us with lunch coupons to stay in the hospital more to learn and reduce our cost of living. Here in this center, there are many arthroplasties arthroscopies of knee and shoulder, and other surgeries, more than enough for fellows to learn. I am also impressed by the team work of the faculties and other staff in the operation theatre. Besides Professor all other faculties, residents and staff are interactive and helpful which has made this fellowship more fruitful. I got the opportunity to witness more than 75( arthroplasties, arthroscopy, ligament reconstruction, osteotomies)knee surgeries, 19 hip surgeries ( arthroscopies and arthroplasties), and 62 shoulder ( arthroscopy and arthroplasty) surgeries in this short span of time. I have witnessed more than 150 patients in clinics. Special thing to mention is here I witness all the techniques ACL reconstruction and all the grafts used for reconstruction mentioned in the textbook. The professor has enrolled us in academic activities by allowing us to participate in a journal club presented by PG students and also given responsibility to prepare the case for presentation in collaboration with PG students.
I got opportunities to scrub and assist in a few cases and expect much more in the upcoming days. If it would be possible to change the fellowship title to “ Arthroplasty and Arthroscopy fellowship” or “ Joint reconstruction fellowship shoulder, knee and hip”., I think the charm of this fellowship will increase more.
Accommodation:
It was difficult to find initially, and the institute didn’t help. I searched for a room through Airbnb. Luckily, I got a very nice landlord, and he is like a local guardian for me. It is around a 25-minute walk from the hospital, but once a week, I have to go to another hospital, which is one hour away by local train.
Cost:
Milan is one of the most expensive cities in the world, so the cost of living is quite high. I am paying 700 Euro per month for a room, around 20 Euro per day(around 500 Euro per month) for transport, daily meals and coffee, and 15 Euro per month for mobile phone recharge. Overall, I am paying around 1200 to 1500 Euro per month for total expenses. This does not include extra restaurants and local tourism.
Conference and networking:
It was of great kindness to the Professor that he registered all of his Trialect fellows to ESSKA Congress 2024 without cost. Not only that, he has also given a special invitation to all the Trialect fellows to the official gala dinner of ESSKA, where I had the opportunity to take photographs with My Professor and Roland Becker, the president of ESSKA. At the dinner, I had the special privilege to dine with Dr. Fillippo Randelli, the Master of hip surgery, Dr. Papa Vasilua, EHPA chair from Greece, and Dr. Vikash Khanduja from Cambridge University. Thanks to Professor for this opportunity and thanks to Trialect.
