This advanced traineeship is aimed at enhancing the operational and expert-level knowledge required for managing complex mechanical ventilation cases. It is designed for clinicians who are already familiar with basic ventilation principles and seek to elevate their practice with advanced techniques in both the ICU and the operating room. The program is built around the latest technological advancements, devices, and evidence-based guidelines in mechanical ventilation.

Advanced Ventilatory Mechanics:

  • Advanced concepts like stress index, mechanical power, and esophageal pressure measurements, which are essential for optimizing mechanical ventilation in critically ill patients.
  • Integration of sophisticated monitoring tools such as volumetric capnography and electrical impedance tomography to assess lung function in real time.

Patient-Ventilator Interaction:

  • Addressing and managing asynchronisms (mismatches between patient breathing efforts and the ventilator) to ensure optimal synchrony and patient comfort.
  • Exploration of advanced ventilatory modes like Proportional Assist Ventilation (PAV) and Neurally Adjusted Ventilatory Assist (NAVA).

Open Lung Concept:

  • Gain expertise in interpreting stress index, mechanical power, and esophageal pressure to assess lung mechanics during mechanical ventilation.
  • Learn to use volumetric capnography and electrical impedance tomography (EIT) to monitor gas exchange and lung ventilation distribution.

Managing Ventilatory Asynchronisms:

  • Identify and treat common ventilatory asynchronisms that negatively affect patient outcomes.

Module 2: Respiratory Physiology Applied to Mechanical Ventilation II

  • Advanced ventilatory mechanics: volumetric capnography.
  • Advanced ventilatory mechanics: electrical impedance tomography.

Module 3: Advanced Mechanical Ventilation I

  • Patient-ventilator interaction: recognizing and managing asynchronisms.
  • Advanced ventilatory modes: Proportional Assist Ventilation (PAV) and Neurally Adjusted Ventilatory Assist (NAVA).

Module 4: Advanced Mechanical Ventilation II

  • The open lung concept: recruitment maneuvers.
  • The open lung concept: PEEP titration trial.

Module 5: Extracorporeal Strategies

  • Extracorporeal CO2 removal (ECCO2R): Indications and management.
  • Extracorporeal membrane oxygenation (ECMO): Understanding the setup and operation for severe respiratory failure.

Learning Objectives

By the end of the traineeship, participants will be able to:

Interpret Advanced Ventilatory Mechanics:

  • Analyze complex ventilatory parameters such as stress index, mechanical power, and esophageal pressure to optimize ventilation strategies.

Diagnose and Manage Ventilatory Asynchronisms:

  • Detect and correct asynchronisms to improve patient outcomes, ensuring better patient-ventilator synchrony.

Apply Advanced Ventilatory Modes:

  • Set and manage advanced assist ventilatory modes such as PAV and NAVA, optimizing them for different clinical conditions.

Implement Open Lung Strategy:

  • Apply recruitment maneuvers and optimize PEEP using the open lung approach to protect both healthy and injured lungs from ventilator-induced lung injury.

Manage Extracorporeal Life Support:

  • Indicate and manage extracorporeal devices such as ECCO2R and ECMO in cases of severe respiratory failure, understanding their setup, indications, and monitoring.

This comprehensive traineeship provides the knowledge and practical techniques necessary for managing complex ventilatory cases, preparing participants to implement advanced strategies in their daily practice.

Week 1:

  • Thursday: 10:00 AM - 12:00 PM
    • Modules 1 and 2: Advanced respiratory physiology and ventilatory mechanics.

Week 2:

  • Thursday: 10:00 AM - 12:00 PM
    • Modules 3 and 4: Advanced mechanical ventilation and patient-ventilator interaction.

Week 3:

  • Thursday: 10:00 AM - 12:00 PM
    • Module 5: Extracorporeal strategies and final course discussion.

This program allows Merit Applications. This program allows merit-based applications for virtual and onsite clinical and research programs. If you are successfully awarded under this category, Trialect or the host mentor will cover the tuition fee only. All applications will be evaluated based on merit. Due to the high level of competition, the chances of being selected under the merit category are quite limited.
