Florida Breast Cancer Foundation

The Florida Breast Cancer Foundation, along with those who generously support us with their talent, time, and resources, is working harder than ever to better the lives of those facing breast cancer in Florida.
We are a nonprofit organization dedicated to ending breast cancer through research, education, and advocacy. In 2002, we along with the state of Florida introduced our specialty End Breast Cancer license plate which allows us to fund even more research and education through the sale of that license plate. Thanks to the continuing support of all Floridians in purchasing the specialty plate. We have invested over $8 million to fulfill our goal of dedicated funding for innovative research projects, educational initiatives, and direct service programs in the state of Florida.
This RFA invites applications from any Florida-based 501 (c)(3) organization providing direct service support to breast cancer patients (see eligibility requirements for active treatment and survivorship).

The Florida Breast Cancer Foundation anticipates awarding grant awards of up to $15,000 maximum each,to organizations to provide living expenses to individuals in active treatment for Breast Cancer, dependenton quality of applications received and budget availability. The aim of this initiative is to reach as manyFlorida breast cancer patients in active treatment as possible.
•The Florida Breast Cancer Foundation anticipates awarding grant awards of up to $15,000 maximum each,to organizations to provide medical items to Breast Cancer patients and survivors, dependent on qualityof applications received and budget availability. The aim of this initiative is to reach as many Florida breastcancer survivors as possible.
•The Florida Breast Cancer Foundation anticipates awarding grant awards of up to $15,000 maximum each,dependent on quality of applications received and budget availability, to organizations providingTomosynthesis (3D) Diagnostic Mammograms or screening MRI services to assist residents of the state ofFlorida in need of these services. The aim of this initiative is to reach as many Floridians in need of 3Dmammograms and/or MRI services as possible.

Important Dates:
October 1, 2024: Announcement of RFA
November 8, 2024: Applications due – All applications must be submitted through ProposalCentral by 5:00 PM EST on this date.
December 2024: Grant Awardees Notified of Funding and Disbursal of Funds
January 1, 2025: Grant Cycle Begins
July 1, 2025: 6-Month Interim Progress Report Due
December 31, 2025: End of Grant Cycle
March 1, 2026: Final Report Due

Restrictions:

Applicants must ensure that all past and current FBCF grants or awards are up-to-date and in compliance with FBCF’s contract and reporting requirements. If the organization has an open Direct Service award with FBCF, the award must be current and due to be completed by the end of the grant period, to be eligible to apply for the new grant cycle. *All no cost extensions requests must be submitted 30 days prior to the grant end date.

Clinical services or patient care services, if requested, must not exceed 100% of the most current local Medicare Part B Allowable Rates for Tomosynthesis (3D) Mammograms or MRI Procedures.

Individual awards to be given to Floridians by grantee are a maximum of $750.00 per person (twice in a calendar year).

Funds cannot be used for: Medical or Scientific Research, Professional Fees, Salaries, Benefits or other Compensation, Facility Fees, Scholarships or Fellowships, Construction or Renovation Charges, Political Campaigns or Lobbying, Endowments, or Debt Reduction.

If the applicant or any of its key employees, directors, officers, or agents are convicted of fraud or any crime involving other financial or administrative impropriety within the preceding 12 months, the applicant is not eligible to apply for a grant during the current funding cycle, and will not be eligible to apply for a new grant until the applicant can demonstrate that appropriate remedial measures have been implemented to mitigate any criminal conduct occurring in the future.

tatement of Need and Funding Priorities:
• Aid breast cancer patients in active treatment (see list of examples).

Aid breast cancer patients and survivors with medical items (see list of examples).
• Provide 3-D mammography diagnostic services for patients with an emphasis on areas of highest need and/or underserved populations, as indicated on the maps.
• Provide MRI screening procedures for patients with an emphasis on areas of highest need and/or underserved populations, as indicated on the maps.
Areas of Interest:
The intention of this award is to provide the (3) funding opportunities that organizations can apply for:
1.
Providing living expenses (see list below) to Florida breast cancer patients in active treatment. Services of interest for this RFA include, but are not limited to:

Travel Expenses for medical appointments

Groceries

Child Care

Rent / /Mortgage

Utilities

Household Cleaning

Yard Maintenance

Providing medical items (see list below) to Florida breast cancer patients and survivors. Medical items of interest for this RFA include, but are not limited to:

Wigs/Toupees

Bras

Prostheses

Lymphedema sleeves
3.
Assisting residents of state of Florida in need of Tomosynthesis (3D) Diagnostic Mammography procedures and/or MRI Screening services
Review: Applications received that are complete and meet compliance with all guidelines will be submitted for review by a panel established through the FBCF Direct Service Committee.
The grant application process is competitive. Whether or not an organization has received a grant in the past, funding in subsequent years is never guaranteed.
Agreements: A grant agreement will be the legal mechanism for funding.
Acknowledgement of Donor Source: Each organization website and project materials must contain an acknowledgment that the funds for the project come from the Florida Breast Cancer Foundation. All printed and electronic announcements for projects and marketing materials for projects must contain FBCF Logo and an acknowledgement that the funds for the projects come from the Florida Breast Cancer Foundation.
Grant period: Grant period shall begin on January 1, 2025 and will conclude on December 31, 2025.
Payment and Reporting: The payment will be made no later than thirty (30) days after receipt of the fully executed agreement. A progress report is due at the end of six (6) months of the start of the grant period and a final report is due no later than sixty (60) days after the completion of the grant period. If reports do not meet the assigned

deadlines, all funds may be requested to be returned to the Foundation.
Letters of support and additional materials: DO NOT send additional materials (i.e. reprints, complete curriculum vitae, or letters of support). Any additional materials will not be reviewed.
Announcement: Announcement of grants awarded will be made in December 2024. Project directors will be notified of the outcome of the review.
Number of grants to be awarded: The Florida Breast Cancer Foundation anticipates awarding the following:
1.
Up to $15,000 maximum each, to organizations to provide living expenses to individuals in active treatment for Breast Cancer.
2.
Up to $15,000 maximum each, to organizations to provide medical items to all Breast Cancer patients and survivors.
3.
Up to $15,000 maximum each, to organizations providing Tomosynthesis (3D) Diagnostic Mammograms and/or screening MRI services.
The actual number of awards for each category is dependent on the quality of applications received and budget availability. The Florida Breast Cancer Foundation reserves the right to award a partial grant for which funds are requested.
Project Narrative:
In the Project Narrative pages of the application, please address the requests below for each section. This is the substance of your application, and the most important sections.
Statement of Need: (limit – 3,300 Characters, including spaces)

Describe the population to be served.

Describe evidence of the risk/need within that population.

Provide statistics specific to the target population.
Project Description (limit – 3,300 Characters, including spaces)

Explain the project’s goals and objectives and how this addresses the needs of the identified population in need of services.

Explain how the project will positively impact Floridians diagnosed with breast cancer who enter, stay in, or progress through the continuum of care.
Budget Justification

Amount of Services Provided: Proposal must specify the type and total number of services, and cost per service to be provided for the requested amount.

Individual awards to be given to Floridians by grantee are a maximum of $750.00 per person.
Proof of non-profit status for applicant institution, must include EIN number (Upload)
All grant applications MUST be submitted in Proposal Central on November 8, 2024 by 5:00 pm.

Important: Failure to meet the above criteria will automatically exclude the grant application from being considered for review and, hence, funding. Any applications received after the deadline will NOT be considered.

 

Only Florida-based non-profit community-based organizations that are tax-exempt under Internal Revenue Code section 501(c)(3), Florida governmental entities, Florida-based tax-exempt educational institutions, and Indian tribes within the State of Florida, are eligible to apply for funding. Organizations registered as 501(c)(3) entities outside of Florida are eligible to apply if a physical presence is maintained within the state of Florida that is open and fully operating for 12 months a year. All activities funded through this competition must occur in Florida. All work must occur, and all funds must be spent in Florida at the applicant organization and any collaborating entities within Florida. Applications must be submitted in English.

Eligible organizations may apply for any of the outlined funding opportunities; however, the grant application process is competitive for each opportunity. Therefore, separate applications must be completed for each opportunity if an organization wishes to apply for more than one. Failure to follow this requirement may result in the organization’s application being administratively rejected.
