The purpose of this Notice of Funding Opportunity (NOFO) is to develop and implement multi-faceted strategies that support the delivery and sustainability of evidence-based chronic disease self-management education (CDSME) programs for older adults with one or more behavioral health conditions.
The two primary goals are:
Goal 1: Develop capacity to increase the number of older adults with one or more behavioral health conditions who participate in evidence-based self-management education and support programs.
Goal 2: Develop and disseminate resources that will enhance the participation of older adults with behavioral health conditions in evidence-based self-management education and support programs.
